1. Walk-in to the Medical Records Counter.
- Location: 1st floor, Counter Number (Admission & Registration)
- Open on Mon-Fri (8:00am - 5:00pm) & Sat (8.30 am - 12.30 pm); Closed on Sun & Public Holidays.
