Pre-Admission and Registration

Self Patients

Admission to Mahkota Medical Centre is only upon the recommendation of a specialist consultant. The specialist consultant will first fill in an Admission Notice. You are to present the Admission Notice, together with your identification card or passport, at any of the Admission and Registration counters located on the Ground, 1st, 2nd, 3rd and 4th floor. You are required to complete an Admission Counselling Form (ACF) and provide details of a guarantor of 21 years or above prior to admission.

You and your guarantor are to provide photocopies both your of identity cards or passports to the Admission and Registration personnel for record purposes.

Based on the admission notice, the Admission and Registration personnel will determine the deposit to be paid on admission. Minimum deposits payable on admission are RM2,000 for admission to normal ward and RM5,000 for admission to the Intensive Care Unit. In the event that the deposit is insufficient to cover the incurred charges, you will be advised to pay extra for the deposit during your hospital stay. A receipt for the deposit paid will be issued for your records.

 

Corporate Patients

Please bring along your guarantee letter, together with your identification card or passport as well as Admission Notice, and present these to the Insurance/ Corporate counter located on the Ground floor. Corporate patients with centralised data are not required to produce a guarantee letter upon admission.

You are required to complete an Admission Counselling Form (ACF). Unless your guarantee letter provides full coverage, you are required to provide a guarantor who is at least 21 years old and pay the relevant deposit on admission. A receipt on deposit paid will be issued for your records.

You and your guarantor are to provide a photocopies of both you identity cards or passports to the Insurance/ Corporate counter personnel for record purposes.

 

Insured Patients

Please bring along your medical card, together with your identification card or passport as well as Admission Notice, and present these to the Insurance/ Corporate counter located on the Ground floor.

The Insurance/ Corporate counter personnel will advise you on your room entitlement. You will be requested to complete an Admission Counselling Form (ACF) and a Declaration & Authorisation form.

Unless the initial guarantee letter provides full coverage, you are required to provide a guarantor who is at least 21 years old and pay a minimum deposit of RM450 on admission.

If you wish to be admitted before initial guarantee letter from your insurance company is received, you will be required to pay a minimum deposit of RM2,000 before admission to normal ward and RM5,000 before admission to the Intensive Care Unit. In the event that the deposit is insufficient to cover the incurred charges, you will be advised to pay extra for the deposit during your hospital stay.

You and your guarantor are to provide a photocopies of both your identity cards or passports to the Insurance/ Corporate counter personnel for record purposes.

 

Discharge

Doctors will certify the discharge during their ward rounds. For your safety, it is advised that a relative or friend accompany you home.

Kindly allow us to have about two hours to process the final bill and to prepare the medication.

For patients covered by insurance, additional time will be required to process the discharge (approximately two more hours).

Please note that if discharged after 2:00 pm, there will be an additional half day room charge; for any discharge after 6:00 pm, an additional day room charge shall be imposed.

The ward clerks will inform patients when the bill is ready to be settled.

Before you leave, please be sure that you:

  • Collect all of your belongings and valuables
  • Obtain all prescribed medication (if any) from the main Satellite Pharmacy (located on the same level as the ward) and ensure that you understand the instructions regarding their use.
  • Receive the appointment card from the attending nurse, should there be any necessary follow-up.
