Admission to Mahkota Medical Centre is only upon the recommendation of a specialist consultant. The specialist consultant will first fill in an Admission Notice. You are to present the Admission Notice, together with your identification card or passport, at any of the Admission and Registration counter located on Ground, 1^st, 2^nd, 3^rd and 4^th floor. You are required to complete an Admission Counselling Form (ACF) and provide a guarantor of 21 years or above prior to admission.

 

You and your guarantor are to provide a photocopy of identity card or passport to the Admission and Registration personnel for record purposes.

Based on the admission notice, the Admission and Registration personnel will determine the deposit to be paid on admission. Minimum deposits payable on admission are RM2,000 for admission to normal ward and RM5,000 for admission to the Incentive Care Unit. A receipt on deposit paid will be issued for your record.

Corporate Patients

Please bring along your guarantee letter, together with your identification card or passport as well as Admission Notice, and present these to the Insurance/ Corporate counter located on the Ground floor. Corporate patients with centralized data are not required to produce a guarantee letter upon admission.

You are required to complete an Admission Counselling Form (ACF). Unless your guarantee letter provides full coverage, you are required to provide a guarantor of 21 years or above and pay the relevant deposit on admission. A receipt on deposit paid will be issued for your record.

You and your guarantor are to provide a photocopy of identity card or passport to the Insurance/ Corporate counter personnel for record purposes.

Insured Patients

Please bring along your medical card, together with your identification card or passport as well as Admission Notice, and present these to the Insurance/ Corporate counter located on the Ground floor.

The Insurance/ Corporate counter personnel will advise you on your room entitlement. You will be requested to complete an Admission Counselling Form (ACF) and a Declaration & Authorisation form.

Unless initial guarantee letter provide full coverage, you are required to provide a guarantor of 21 years or above and pay a minimum deposit of RM350 on admission.

If you wish to be admitted before initial guarantee letter from your insurance company is received, you will be required to pay minimum deposit of RM2,000 before admission to normal ward and RM5,000 before admission to the Incentive Care Unit.

You and your guarantor are to provide a photocopy of identity card or passport to the Insurance/ Corporate counter personnel for record purposes.
