Pre-Admission and Registration


Admission to the Centre is permissible only upon the recommendation of a doctor. The attending doctor will first fill in an Admission Notice Form (ANF). The patient will then have to proceed to the registration counter and present the form to the attending registration executive. It shall be requested that an Admission Counselling Form (ACF) be completed prior to admission.

With the details listed on the ANF, the registration executive will provide you with an estimate of the particular treatment and determines the deposit amount required. After the deposit has been paid, the registration executive will issue a receipt for your record.

All admissions require a guarantor (21 years or above) to sign a letter of undertaking. In addition, the patient (if physically and mentally able) shall also be requested to sign a letter of undertaking to pay. Both, the guarantor as well as the patient will need to pass his or her identity card (IC) or passport to the registration executive to photocopy.

After the completion of the ACF, please submit this to the registration executive who will be able to finalize the registration process.

Corporate Patients

For those with corporate accounts under Mahkota Medical Centre, please bring along your guarantee letter and pass this to the registration executive at the registration counter. Corporate patients with centralized data are not required to produce a guarantee letter upon admission. All corporate patients shall be requested to complete an Admission Counselling Form (ACF). Upon completion, please submit this, together with your identification card (IC) to the registration executive.

Should the patient request for additional entitlements, or should the total procedure cost exceed that of the guaranteed amount, a deposit shall be collected.

Insured Patients

Please bring along your insurance card and pass this, together with your identification card (IC) on to the registration executive who will verify the information. You will be requested to complete an Admission Counselling Form (ACF) and provide a RM350 deposit as a contingency for excess charges over entitlements.
